http://www.camft.org  is the URL for the California Association of Marriage and Family Therapist.  At this site you can join CAMFT as a student member and obtain your malpractice insurance.  We require that all trainees have  malpractice insurance before beginning their hours.

http://www.aamft.org/index_nm.asp is the URL for the American Association for Marriage and Family Therapy. AAMFT is the national representative for MFT's.  They are directly involved in obtaining provider ship for MFT's on the National Level. 

http://www.bbs.ca.gov is the URL for the Board of Behavioral Science which is responsible for the licensing of Marriage, Family and Child Counselors aka Marriage and Family Therapist.

You must go to this site and print these forms for your use. The filling out of these forms is the responsibility of the trainee/intern.  Your student MFT trainee hours required for each course  completion will be verified on the Weekly Summary of Hours of Experience. 

A student trainee must have 1 hour of individual or 2 hours of group supervision for every 5 hours of direct patient contact. Each practicum reflects 96  hours - a minimum of 80 client contact 
hours
and a minimum of 16  hours individual supervision  or 32 hours of group supervision  -  All three 688, 689, and 690 require  240 hours of direct patient contact and 48 hours of individual supervision or 96 hours of group supervision.

A student will not be allowed to enroll in the next practicum without having completed 96 hours in 688 and 196  hours by the end of 689. Pending satisfactory progress, a student will receive an SP
(satisfactory progress) grade until a comprehensive total of 96 hours is completed for 688, 96 hours is completed for 689, and 96 hours is completed for 690. At the point that the required number of hours is achieved, the grade for that particular practicum course will be changed to a Pass (P).

MEDLINE is the index of the National Library of Medicine the largest periodical database in the world. All of the psychological and counseling journals are contained within this library.

Course Description: Each practicum reflects 150 hours - a minimum of 125 client contact hours and a minimum of 25 supervision hours - ten of which must be on-site individual supervision and 15 as group supervision. In addition there will ten hours of individual and group supervision provided by the Chapman University Practicum Instructor.

A student will not be allowed to enroll in the next practicum without having completed 125 hours in 688 and 275 hours by the end of 689. Pending satisfactory progress, a student will receive an SP (satisfactory progress) grade until a comprehensive total of 150 hours is completed for 688, 300 hours is completed for 689, and 450 hours is completed for 690. At the point that the required number of hours is achieved, the grade for that particular practicum course will be changed to a Pass (P).

Course Objectives:

1. Integrate counseling theory with practice.

2. Integrate counseling skills in individual, group, marital, child, and family counseling.

3. Maintain, in writing, confidential clinical case notes and files, including a five axial 
diagnosis and expected clinical goals, prognoses, and treatment outcomes.

Written case presentation suitable for publication..

A: Professional Practice Statement of Policy and Procedures: -688

I. Statement of counseling practice. This includes your
philosophy of counseling, therapeutic approach and the
techniques that you use.

II. Fees for service. missed appointments, fee scale, late
appointments, appointment cancellation procedure, payment of
fees, and termination of therapy.

III. Contract for services:

IV. Confidentiality statement

V. Confidentiality release form to receive and to export
confidential information.

VI. Brief resume.

VII. Clinical Notes

VII. DSM IV - Multiaxial Diagnosis Form.

VIII. Treatment Plan

In addition to the statement of policies and procedures the student will provide written 
justification of same.

B: Personal Journal - 688-689-690

The student will also maintain a personal journal of their feelings, thoughts, and 
experiences in providing counseling, their supervision experiences, and their personal 
therapy.

C: Clinical Notes and Oral Case Presentation - 688, 689, 690

The student will keep clinical notes of their patients progress including diagnosis and treatment plan to be reviewed with their supervisor. Use the attached Services and Payment Log, Progress Notes, and Closing Note forms.

D: Written Case Presentation - Suitable for Publication Research Paper: 688, 689, 690 -

The student will select a patient (indivdual or group or family) in 688 and research the diagnois and treatment for the presenting problems identified in the assessemnt. The research paper will include:

II. Abstract

III. Table of contents

IV. Introduction, Statement of the problem (Presenting problems, Diagnosis, Etiology), Statement of Hypothesis as applied to diagnosis and treatment.

V. Review of current literature. (You must have a least 20
citations of recent work post 1990.)

Grading:

A: 30% for statement of policies and procedures and
justification.

B and C -30% for personal journal of counseling experience, clinical notes, oral 
case presentation.

D: 30% for written case presentation - research paper

E: 10% for class participation.

Course Outline:

Session 1: Review course outline: personal journal, policies and procedures, 
clinical notes, research paper. Select research topic: discussion of present 
caseload.

Session 2: Discussion of counseling issues and observed sessions.

Session 3: Discussion of journals, patients, taped sessions.

Session 4: Presentation of sessions and tapes. Term paper, personal journals, policies 
and procedures , clinical notes due
